Assertive Community Treatment Team (ACTT)
Team-based, community-delivered care for adults with severe and persistent mental illness. ACTT serves as a single point of responsibility and offers 24/7/365 support with services provided in homes and community settings. Intended for adults with significant functional impairment and a history of crises, hospitalization, homelessness or incarceration; not intended for primary SUD, IDD, TBI or autism as the primary diagnosis.
Comprehensive cognitive-behavioral treatment for adults with borderline personality disorder and severe emotion dysregulation. DBT combines individual therapy, skills group, therapist consultation and between-session coaching. Members practice skills (e.g., diary cards) to reduce self-harm, crises and high-risk behaviors.
Individual psychotherapy for posttraumatic stress disorder and trauma-related symptoms. Delivered by EMDR-trained clinicians, EMDR uses structured protocols and bilateral stimulation (e.g., guided eye movements) to help reprocess traumatic memories. The number of sessions varies by clinical need; adaptations exist for youth.
Intensive, evidence-based, in-home therapy that strengthens safety and stability for at-risk children and adolescents and their families. FCT supports reunification, reduces out-of-home placement, and is delivered by trained professionals under certified supervision. The model is trauma-informed and individualized.
Intensive, team-based, family-driven planning and care management for youth/young adults with complex needs who are involved in multiple systems or at risk of higher levels of care. The HFW team (facilitator, family partner, youth partner, supervisor/coach) builds a strengths-based plan, coordinates services and supports transition.
Home-based, intensive treatment for youth with serious behavioral problems who are at risk of out-of-home placement or returning from placement. MST engages family and key systems (school, justice, community), typically over 3–5 months, with small caseloads and 24/7 therapist availability. MST-PSB adapts the model for problem sexual behavior.
Recovery-oriented, community program for adults with psychiatric disabilities to build skills for education, employment, daily living and social connection. Clubhouses use a work-ordered day with members and staff operating the program together. PSR is not available to individuals receiving ACTT.
Short-term, structured therapy for children and adolescents experiencing trauma-related symptoms. TF-CBT integrates caregiver participation, skills training and a trauma narrative to reduce distress and improve functioning across home, school and community.
