Issued:  October 25, 2019 Providers:  Information Requested by October 31, 2019  Partners requests that providers complete a brief survey by October 31 to assist in the completion of Annual Performance Reviews (APR). Per Partners Provider Operations Manual criteria and our monitoring responsibilities, account specialists complete an APR for all contracted providers. Completed APR results are reviewed by the Network Management Committee.  A score of 75% on the APR
