2024 Perception of Care Survey Update
Thank you for your ongoing support in completing the Perception of Care Survey. The survey links are live now until Friday, Sept. 20, 2024, unless an extension is granted by the state. See the necessary information for administering the survey to members below. This information is also on Partners’ Perception of Care webpage.
To meet our target, we need:
- 368 adult surveys
- 174 youth surveys
- 104 child family surveys
This survey targets populations with mental health and substance use disorders (SUD) and is federally mandated for behavioral health services in North Carolina. The data will help identify areas for improvement in member engagement and services.
Providers should review the 2024 Perceptions of Care Provider Survey Administration Guidelines for instructions on assisting members with the surveys.

Tailored Plan Member and Care Manager Information Now Available
As part of a state requirement, Partners Health Management is offering providers a list of their Partners Behavioral Health and Intellectual/Developmental Disabilities Tailored Plan members who are eligible for or receiving care management under Partners Tailored Plan. A list of Partners’ tailored care managers is also available.
Providers can request this information by contacting Partners at 888-235-4673, option 5.
Tailored Plan is a specialized North Carolina Medicaid health plan designed for individuals with intellectual/developmental disabilities (I/DD), substance use disorders (SUD) or traumatic brain injury (TBI). It covers physical health, behavioral health, I/DD, pharmacy and non-emergency transportation services. As part of this plan, the Health Insurance Portability and Accountability Act (HIPAA) care coordination exceptions apply, meaning a Release of Information is not required for sharing relevant care management details.
How Tailored Care Management (TCM) Benefits Patients and Providers:
- Person-Centered Assessment: Tailored care managers conduct in-depth assessments to address medical, social and psychosocial needs, creating individualized care plans.
- Chronic Disease Management: Care managers monitor chronic conditions, provide education and support self-management to improve health outcomes.
- Transitions of Care: Support is provided during transitions such as hospital discharges to ensure follow-up care and reduce readmissions.
- Care Management: Care managers facilitate communication among providers, ensuring necessary referrals and continuity of care.
- Behavioral Health Support: Tailored care managers collaborate with mental health providers to address behavioral health needs.
For more information, contact Partners at 888-235-4673, option 5.

REMINDER: Guidance for Transitioning from 1915(b)(3) to 1915(i)
The transition from 1915(b)(3) services for NC Medicaid Direct and the Behavioral Health and Intellectual/Developmental Disabilities Tailored Plan members will be completed by Dec. 31, 2024. For detailed guidance, review the Partners Provider Alert issued on June 28, 2024 and Partners Provider Communication Bulletin #155.
Medicaid Direct Benefit Plan:
- The N.C. Department of Health and Human Services’ (NCDHHS’) authority to provide 1915(b)(3) services under the 1915(b) waiver ends Dec. 31, 2024. Partners will not authorize any 1915(b)(3) services for its members after Dec. 31, 2024. Authorizations with end dates beyond Dec. 31, 2024, will have the end date adjusted by Utilization Management (UM) to Dec. 31, 2024.
- Members MUST be assessed for eligibility for 1915(i) services by the Tailored Care Manager (TCM) or care manager, as soon as possible. The 1915(b)(3) providers should reach out to the TCM provider or care manager to support development of the Care Plan/Individual Support Plan (ISP).
- Failure to work with the TCM provider or Care Manager to support the development of the Person-Centered Care Plan/Individual Support Plan (ISP) and complete transition to 1915(i) or another appropriate service by Nov. 30, 2024, may result in service disruption for the member on Jan. 1, 2025.
Tailored Plan Members above the 150% Federal Poverty Level (FPL):
- Presumptive eligibility cannot be given to members above 150% FPL.
- Any member who has not been assessed in the last 12 months must be reassessed prior to independent evaluation and determination of 1915(i) eligibility.
- NCDHHS will provide Partners with a list of members with income over 150% FPL and whose assessment is more than 12 months old prior to July 1, 2024. This will allow the Tailored Care Manager or Partners care manager to coordinate with providers to complete the member’s reassessment and develop the Person-Centered Care Plan/ISP.
- Members already receiving 1915(b)(3) services can continue to receive 1915(b)(3) until the 1915(i) Care Plan/ISP is approved and services are authorized.
- If these members are receiving 1915(b)(3) services, they must be transitioned to 1915(i) or another appropriate service or otherwise stop 1915(b)(3) services no later than Sept. 30, 2024.
- UM will end all 1915(b)(3) authorizations for Tailored Plan members on the FPL list on Sept. 30, 2024.

Provider DEI Collaborative – Quarterly
Oct. 10, 2024, 2-3 p.m. (Register)
The purpose of this quarterly collaborative facilitated by Vickie Smith is to bring awareness and increase conversations related to the topics of diversity, equity and inclusion (DEI) related to our provider network.
Providers, we would like your participation in this collaborative! Your perspective is vital for health equity initiatives for the members we serve.
The goal of the DEI Provider Collaborative is to ensure that providers have access to the needed resources to serve members through the lens of ensuring diversity, equity and inclusion and to foster better outcomes and improve quality of life for members. To achieve these goals, we must know and understand the different populations we serve.
As we transition to whole person integrated care of our members, it is important that we create actionable strategies to address members’ behavioral, physical and pharmacy care. Consistent provider engagement in these discussions is critical to members’ overall quality of care.
Please register here or on the Partners TRAC page at PartnersTraining.org, and let’s get to work!
