Authorization Tips for CGM Devices
To help avoid delays in access and reduce administrative work caused by denials and appeals, Partners Health Management is providing key reminders for completing prior authorization (PA) and reauthorizations for Continuous Glucose Monitoring (CGM) devices, including Dexcom and FreeStyle Libre systems.
There are three distinct authorization phases for CGM devices:
- Initial Authorization—valid for 6 months

- Subsequent Reauthorizations—valid for 12 months
Each phase has specific documentation requirements related to:
- Patient diagnosis
- Use of an external insulin pump
- Face-to-face evaluations
- Evidence of glycemic control
For more detailed guidance, including age-specific coverage and documentation requirements, view the Continuous Glucose Monitoring (CGM) Devices: Prior Authorization and Reauthorization Guidelines.
