Prevention and Population Health Programs

Partners believes in Improving Lives and Strengthening Communities. We offer programs and resources to help members improve their overall health and well-being.

Our population health programs target common concerns such as:

  • Managing chronic conditions
  • Healthy pregnancies
  • How to stop using tobacco

To request additional information or to refer a member to any of our population health programs, email PopulationHealthPrograms@PartnersBHM.org.

Population Health Assessment

Partners conducts an annual population health assessment to better understand our members. This assessment reviews demographics, the needs of specific member groups and patterns of service use.

The 2026 Population Health Assessment provides valuable insights that help measure the impact of Partners’ population health strategies and guide future planning.

Partners’ Pregnancy Program

Partners is committed to supporting our members at every stage of life. Our pregnancy program offers help to members who are pregnant. People who identify their pregnancies early and receive appropriate health care are less likely to have serious complications and healthier outcomes. Through care management and support, this program ensures expecting members receive the best possible care.

Here’s how providers can support Partners’ Pregnancy Program:

  • Providers, care managers or caregivers can refer members to the program. Members may also self-refer. Email referrals or questions about the program to: PopulationHealthPrograms@PartnersBHM.org
  • Educate Partners’ pregnant members at PCP and specialty doctor visits on the resources offered.
  • Refer members to CMHRP.
  • Encourage members to reach out to their care managers for more information.
