NC-TOPPS Overview
North Carolina Treatment Outcomes and Program Performance System (NC-TOPPS) is a statewide, web-based program that collects outcome and performance data for individuals receiving mental health and substance use services in North Carolina’s public service system.
The system is designed to:
- Measure the impact of treatment
- Support clinical decision-making
- Improve service quality across the network
Data is collected through interviews conducted face-to-face between a member and a Qualified Professional. These interviews help inform treatment planning, track individual progress and support system-wide quality improvement.
Provider agencies are responsible for completing and submitting NC-TOPPS interviews. Partners’ Quality Management team monitors submission activity monthly and offers training and support to agencies that fall behind.
All official communications from NC-TOPPS are sent directly to the primary provider agency NC-TOPPS contact, also known as a Superuser.
Kudos Corner
The NC-TOPPS Kudos Corner celebrates agencies across our network that demonstrate excellence in timely submissions, completed interviews and data-driven care.

Transferring a Member’s NC-TOPPS History
When a member transfers from one provider agency to another to continue services, their NC-TOPPS history may need to be transferred.
To initiate a transfer:
- Complete the Authorization for Use, Disclosure and Exchange of Protected Health Information for NC-TOPPS form
- Include the full date range for the member’s most recent episode of care
- Fax the form to Partners or email it to NCTOPPSQuestions@PartnersBHM.org
Example: If a member began services with Provider A on 12/10/2020, the date range can be “12/10/2020 to present” or simply “2020 to present.”

When should I request a member’s NC-TOPPS history transfer?

To request a transfer:
- Contact the Partners Help Desk for the authorization form
- Include the correct date range (e.g., “12/10/2020 to present”)
- Ensure the form is complete and legible to avoid delays
Do I need to submit an Episode Completion interview for an out-of-state transfer?
Yes. If a member is transferring to a provider outside of North Carolina, an Episode Completion interview is required.
What if a member receives more than one qualifying service?
Refer to the hierarchy chart in the NC-TOPPS Guidelines or FAQ document to determine which service takes reporting priority.
Is a Superuser required for my agency?
Yes. Every provider agency must designate at least one Superuser. Superusers are responsible for:
- Managing NC-TOPPS users at their agency
- Monitoring submission activity
- Ensuring timely completion of interviews
Without a Superuser, user access requests will remain pending. To become a Superuser, request an enrollment form from either the state NC-TOPPS team or the Partners Help Desk.
