Reminder: 2026 Perception of Care Survey Closes Sept. 18

The 2026 MH/SUD Client Perceptions of Care Survey closes Sept. 18, 2026. All surveys are required to be submitted electronically via the appropriate link for the member’s population (Adult, Youth, Child/Family). Spanish versions are also available.

What is happening?

Partners Health Management is requesting providers of mental health and substance use disorder services (MH/SUD) to complete as many surveys as possible. We need the help of all providers who had members in MH/SUD services between July 1, 2025, and June 30, 2026, to meet the target provided by the North Carolina Department of Health and Human Services (NCDHHS).

Our goal is to have 700+ adults, 200+ youth, and 125+ child and family surveys completed for each population.

What do providers need to do?

Our focus is on all enhanced services such as intensive in-home (IIH), Assertive Community Treatment Team (ACTT), residential, and substance use disorder (SUD) services. This includes Transitions to Community Living (TCL) services that are not captured in other enhanced services.

For surveys to be counted, the following must be included:

  • Provider location NPI (not individual clinicians/corporate numbers)
  • Member Alpha number (not internal provider ID)
  • Member Medicaid number

Navigate to the Community MH/SUD Client Surveys Provider Page.

Optional: Before beginning to administer the surveys or enter response data, you can navigate through the web-based surveys to get familiar with the layout. Select any survey from the main Client Survey Provider Page, then enter test responses to the provider questions on Page 1 and 2 before paging through the rest of the survey.

Reminders will be sent throughout the survey period, as Partners receives weekly updates from NCDHHS on our numbers.

Who to contact?

Partners’ survey coordinator is Keehana Avinger. If providers have questions, they can email her at kavinger@partnersbhm.org.
