The North Carolina Department of Health and Human Services (NCDHHS) has posted an updated version of the Record Management and Documentation Manual (RMDM). The updated manual was posted on July 8, 2025, to the NCDHHS website.

• Effective July 8, 2025, the NCDHHS posted an updated Record Management and Documentation Manual.
• The updated RMDM provides guidance on records administration, clinical service records, evaluations, service notes, and more.
