Issued: January 24, 2025

The North Carolina Department of Health and Human Services (DHHS) and the Division of Information Technology staff will be performing an update to the IRIS System on Monday, January 27. The system is expected to be unavailable from 12:00 p.m. to 12:15 p.m. that day.

For additional support:

  • TPs may contact DMH/DD/SUS, and PHP staff may contact DHB with any policy-related questions.
  • Providers with technical IRIS-related questions may contact the DMH/DD/SUS Customer Service and Community Rights Team at 984-236-5300.

We apologize for any inconvenience this may cause.

Important Reminders:

Since IRIS reports must be submitted within 72 clock hours of the provider learning of an incident, providers may use the following methods during the maintenance window:

Submission Instructions:

  • Reports can be faxed to the TP (Host and Home) and to DMH/DD/SUS Customer Service and Community Rights Team at 984-777-9264 within the required timeframes. Providers will still need to enter the report into IRIS once the system is back online.
  • For reports to DHSR Healthcare Personnel Registry, fax them to 919-733-3207 within the specified timeframes.
  • For deaths related to suicide, homicide/violence, accident, or restraint in a licensed facility, please report to the DHSR Complaint Intake Unit via fax at 919-715-7724.

For further assistance:

Providers may contact Partners Health Management at iris@partnersbhm.org or call Member Engagement at 877-864-1454.
