Partners Health Management has learned that the Department of Health Service Regulation (DHSR) Complaint Intake and Health Care Personnel Investigations Sections will be relocating on Thursday, April 24, 2025. To support the move, fax lines and machines will be disconnected on Wednesday, April 23, 2025, and DHSR will be unable to receive faxes during that time. This includes Initial Allegation and Investigation Reports related to abuse, neglect, misappropriation, fraud, or drug diversion.
The duration of the fax outage is currently unknown, as it depends on external vendors. DHSR Licensure and Certification Sections have been notified that providers may experience delays in transmitting reports on April 23.
Please note: DHSR Telephone lines will remain active, and the mailing address will not change.
