Partners Health Management is aware that some providers may be experiencing service disruptions due to the impact of Hurricane Helene. To assist these providers, Partners has created tools to report and track service and financial disruptions.
Providers can notify Partners of service disruptions to members via the Temporary Disruption to Service Delivery form. Financial reimbursement requests may be submitted via the Disaster Expense Reimbursement form. Upon receipt of the forms, Partners will review and process them as quickly as possible.
Both forms should be completed and emailed to pas@partnersbhm.org Please reach out to your assigned provider account specialist or email pas@partnersbhm.org for updates on this process.
