The 2025 MD/SUD Perceptions of Care Survey links are live from Monday, Aug. 11, 2025, through Friday, Sept. 19, 2025. Survey administration options are the same as last year to include paperless surveys and distance technologies. All surveys must be submitted electronically by Saturday, Sept. 20, 2025. Below is the information providers need to administer the survey to members. This information is also on Partners Health Management’s website for your reference.
What’s happening?
North Carolina Department of Health and Human Services has released its annual MD/SUD Perceptions of Care Survey. Links are live from Monday, Aug. 11, 2025, through Friday, Sept. 19, 2025.
This survey is for Mental Health and Substance Use Disorder populations. It is federally required for the state of North Carolina behavioral health services. The outcomes data will help identify areas of opportunities to improve engagement and services for members.
Partners needs the help of all MHSU providers to participate in this year’s administration of the perception of care survey to members who received services between July 2024 and June 2025. Our goal is to have 700 adults, 200 youth and 125 child and family surveys submitted.
Surveys must be submitted electronically by Friday, Sept. 19, 2025.
What do providers need to do?
Providers need to access the 2025 MD/SUD Perceptions of Care Survey Administration Guidelines for Providers for Providers and read about helping members complete the surveys.
For the survey to be counted as completed, provider location NPI (not individual clinicians/corporate numbers), member Alpha number (not internal provider ID) and Medicaid number must be included in the first part of the survey.
Providers should click on this link to access the surveys: Community MH/SUD Client Surveys Provider Page and have as many clients as possible complete the surveys. (Examples of services include IIH, Residential, ACTT and SUD. Enhanced services are preferred, but not required.)
