Each year, Partners conducts a Community Network Adequacy and Accessibility Analysis to understand the health needs of the citizens in the regions we serve. To assist with this process, we want input from every perspective and every person touched in some way by mental illness, substance use disorders, traumatic brain injuries or intellectual and developmental disabilities.
The survey should only take 5 to 10 minutes to complete. Want to participate?

- discussions with community and provider stakeholders
- wide-ranging data analytics
- web-enabled survey
